The following immunizations must be completed within the first two months of school.
Medical exemptions to these requirements can be explained by the school nurse.
The Diocese of Philadelphia does not recognize religious exemptions to immunizations.
Required for All Students
- Diphtheria, Tetanus, and Acellular Pertussis (DTaP)
Five doses of vaccine (fifth dose on or after fourth birthday) - Polio (IPV)
Four doses of vaccine (fourth dose on or after fourth birthday and at least six months after previous dose) - Measles, Mumps, and Rubella (MMR)
Two doses of vaccine - Hepatitis B
Three properly spaced doses of the vaccine - Varicella (Chicken Pox) Immunity
Two doses of the vaccine or evidence of immunity (history of chicken pox or laboratory testing documentation)
Additional Requirements for Students Entering Grade 7
- Tetanus, Diphtheria, and Acellular Pertussis (Tdap) — one dose
- Meningococcal Conjugate Vaccine (MCV) — one dose
