Required Immunizations

The following immunizations must be completed within the first two months of school.  Medical exemptions to these requirements can be explained by the school nurse.  The Diocese of Philadelphia does not recognize religious exemptions to immunizations.

  1. Diphtheria, Tetanus, and Acellular Pertussis (DTaP): Five doses of vaccine (fifth dose on or after fourth birthday)
  2. Polio (IPV):  Four doses of vaccine (fourth dose on or after fourth birthday and at least six months after previous dose)
  3. Measles, Mumps and Rubella (MMR):  Two doses of vaccine
  4. Hepatitis B:  Three properly spaced doses of the vaccine
  5. Varicella (Chicken Pox) Immunity:  Two doses of the vaccine or evidence of immunity (history of chicken pox or laboratory testing documentation)

In addition, students entering Grade 7 need the following vaccines.

  1. One dose of Tetanus, Diptheria,  acellular Pertussis (Tdap)
  2. One dose of Meningococcal conjugate vaccine (MCV)
